independiente (SAVI) est\u00e1 dirigido a personas con discapacidad intelectual y\/o con discapacidad derivada de enfermedad mental que quieren vivir de forma autogestionada e independiente en una vivienda propia o de alquiler, solas, en pareja o con otras personas que tengan reconocido un grado de dependencia igual o superior al grado I.<\/span><\/p>\n<p><strong><span style=\"color: #e98270;\">\u00bfC\u00f3mo lo hacemos?<\/span><\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Contamos con un equipo multidisciplinar, comprometido con las personas y su integraci\u00f3n, que las acompa\u00f1a en la toma de decisiones y las dota de herramientas y estrategias para que puedan vivir de forma independiente.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Se elabora anualmente un plan de atenci\u00f3n personal, de manera conjunta con la persona, en el que se valoran las \u00e1reas de necesidad de apoyo teniendo en cuenta sus habilidades y competencias.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Se hacen visitas semanales al domicilio de la persona para llevar a cabo un seguimiento personalizado.<\/span><\/li>\n<\/ul>\n<p><strong><span style=\"color: #e98270;\">\u00bfA qui\u00e9n va dirigido?<\/span><\/strong><\/p>\n<p><span style=\"font-weight: 400;\">A personas de entre 18 y 65 a\u00f1os que tengan reconocido un grado de discapacidad igual o superior al 33%. seg\u00fan evaluaci\u00f3n del Equipo de Valoraci\u00f3n y Orientaci\u00f3n (EVO) del Departamento de Trabajo, Asuntos Sociales y Familias (DTASF), y con trastornos de conducta y necesidades de apoyo
